St. Peter's Health will be conducting sports physicals at Jefferson High School on May 10 and 11 from 4:30pm to 7pm. The physicals will cost $20 and all of the money will be returned to the JHS athletics program! This is a win-win for everyone. It's an easy way to get your physical and also put some money back into athletics.

 

The incoming 8th graders are also welcome to participate!

 

Please make sure to bring this MHSA form with you. It must be printed and signed by a parent or guardian. The form is available at the front office if you don't want or are unable to print it at home.

 

The activity bus will be available for any student who stays to get a physical either of the two days. 

 

Appointments are not required; however, please inform the school if you plan on taking advantage of this opportunity so we can give St. Peter's Health an estimate of how many students are coming. If you have any questions, please call JHS at 225-3317.

Cost: $20 payable to St. Peter's Health (cash or check). Please note: 100% of these fees will be returned to the JHS athletic fund!
